How do you track your progress?

In terms of making any changes for your health be it diet, taking supplements, or working out - How do you track your progress? 

Everyone tells me to use an app for this but I am not glued to my phone like most other people nor to I enjoy using apps. I still use a physical planner. lol

I use a physical planner as well and actually use that for everything. I have water tracking stickers to ensure I am drinking enough water as well as mood stickers, ect. you can just buy planner stickers to track your goals.
